We write with important information about your upcoming COVID-19 vaccination appointment. COVID-19 Vaccinations are only available to those who meet federal and state criteria. Currently, the County of Santa Clara is only vaccinating (1) healthcare personnel who live or work in Santa Clara County and (2) residents of Santa Clara County who are 75 and older. When you arrive for your appointment, you will be turned away if you don’t meet these criteria. Any information you may have received suggesting that appointments are available for persons not meeting these criteria was incorrect. The County issued a media advisory regarding misinformation about vaccine appointment availability that is available here.

When you arrive for your appointment, you will be asked to provide identification, proof of eligibility such as a government-issued photo ID with your date of birth or documentation confirming you are a healthcare workers such as an ID badge/card from your work, a paystub, or other documentation from your employer. You will also be asked to sign an attestation, under penalty of perjury, that you meet criteria.
If you do not meet the criteria above, please immediately cancel your appointment so that other eligible members of our community can be vaccinated. Information on who is eligible to be vaccinated, and updated information on vaccine availability, is available at our website: www.sccfreevax.org. If you aren’t currently eligible to be vaccinated, we encourage you to sign up as soon as you are eligible.
